REGISTERED NURSE INTAKE COORDINATOR

Position Summary
The primary purpose of this position is to support the overall operation of patient care areas. The RN Intake Coordinator assesses, plans, implements, coordinates, monitors and evaluates options to facilitate the continuum of care. It also provides a full range of case management services in collaboration with a multidisciplinary team. The RN Intake Coordinator assist in appropriate and timely placement of admitted patients, as well as transfers. The RN Intake Coordinator educates staff regarding effective allocation of the hospital’s resources while encouraging provision of high-quality patient care and is cognizant of regulations and policies of all review organizations relating to billing of the fiscal intermediaries. Participates in Case Management Team activities and responsibilities.
Primary Responsibilities

1. Collaborates with medical staff, Administration, and other managers in planning, directing, coordinating, providing, improving, and integrating healthcare services that are responsive to community and patient needs and that improve patient outcomes.
2. Assists in appropriate and timely placement of admitted patients, by reviewing clinical data using InterQual Criteria.
3. Assists in the transfer process of patients.
4. Assists Unit Managers in tracking of current census throughout the hospital.
5. Guides appropriate use of all resources for efficient, safe patient care.
6. Participate in daily multidisciplinary clinical case conference.
7. Coordinate discharge arrangements if needed.
8. Maintains current knowledge of Case Management and Nursing issues through continuing education.
9. Daily rounds through all patient-care departments to gain information from patients, staff, and physician.
10. Maintaining visibility to patient-care units to emphasize the team over the individual, and nurture positive relationships with staff.
11. Provides guidance to clinical personnel as needed.
12. Effectively and consistently communicates administrative/managerial directives to personnel.
13. Aware of regulatory requirements, including all state, federal and TJC regulations related to the hospital.
14. Responds appropriately to emergency situations, including fire, disaster, and clinical emergencies.
15. Utilizes the risk management channels for reporting hazards and adverse occurrences.
16. Participates in organization-wide performance improvement activities.
17. Complies with all organizational policies.
18. Assures confidentiality of information pertaining to patients, staff, and hospital business.
19. Other job duties that may be assigned.
Competencies
- Basic computer knowledge.
- Ability to use appropriate discretion in handling of confidential information and materials.
- Extensive knowledge of department and hospital policies and procedures.
- Interpretive skills required for frequent interactive and consultative associations.
- Good management and supervisor skills preferred.

Requirements:
Required Education and Experience
1. Must have graduated from an accredited school of nursing; BSN preferred, or may substitute equivalent in experience.
2. Current State of Louisiana Registered Nurse Licensure or compact license.
3. Two years of nursing experience required.
4. Current BCLS certification or must be obtained within 3 months of hire.
Preferred Education and Experience
- Case Management Experience preferred.
- Current ACLS certification preferred.
